Distal biceps tendon rupture

Biceps tendon torn from the forearm bone at the elbow.

What it is

The biceps tendon can pull off its attachment at the elbow, usually when a heavy weight is caught unexpectedly. This is a time-critical injury: repair within a few weeks is much simpler and restores more strength, particularly for turning the palm upwards.

How it usually presents

  • A painful pop at the front of the elbow with bruising
  • The muscle bunching upwards in the arm
  • Marked weakness twisting a screwdriver or turning a door handle

How it is assessed

Clinical examination and ultrasound or MRI, arranged promptly.

Treatment options

  • Early surgical repair for most active patients
  • Non-surgical management where surgery is not appropriate, accepting some loss of strength
